My [22/F] boyfriend [20/M] is really lazy and it's a constant battle to get him to even do a load of dishes. What do I do?

So I've been dating my boyfriend for a year, but we have been friends for much longer. We live together. He goes to university and works, but that totals maybe 24 hours a week. He is also congenially disabled; he is missing his left arm from the elbow down since birth. He has shown me it doesn't hold him back but it may be a factor in this.

I currently stay at home as I am deemed unfit to work by doctor/therapist due to severe mental health issues. Basic life is hard for me. I can hardly remember to eat, shower, or even find motivation to get out of bed. I struggle daily, but I never ask my boyfriend to do any chores. I do all the cleaning, I wash his clothes and keep everything tidy, manage our finances and pay bills, do grocery shopping and all the cooking. All that I ask him to do is wash the dishes after dinner, so I can make food for him the next day. He usually doesn't even do them all or does a bad job, too.

He doesn't really do anything, it's beginning to break me. I have to literally fight him on it, even though we agree agreed to this months ago. I am getting so frustrated. I went away on a trip for a week and he had ordered nothing but takeout without me, there was moldy food everywhere, the apartment was a complete disaster and I spent the evening I got home crying my eyes out and frantically trying to damage control the absolute pig sty he managed to make when I had cleaned everything from top to bottom a week prior.

I am at a breaking point, he doesn't seem to understand that helping me with chores is an expression of love and I am very sick, he is causing me unnecessary stress and it's not helping. I don't know what to do.

Tl;dr: I am very mentally ill and struggle to take care of myself, I do every chore in our home besides doing dishes after I cook dinner. Have to basically battle my boyfriend if I want clean dishes for the next night. Beginning to break me in the relationship. Don't know what to do.
